Our doctors are able to put a GP Management Plan or Care Plan in place for patients with chronic conditions such as Asthma, Diabetes, Heart Disease, Arthritis as well as other conditions.
A Care Plan acts as a central document where we can keep all the information about how to best manage your condition, as well as what other health providers are involved in your care, all in the one document.
One of the great things about having a Care Plan is that it may give you access to up to 5 Allied Health Sessions such as physiotherapy, podiatry, dietetics, exercise physiology, chiropractors and osteopath sessions which are partly paid for by Medicare.
Getting a care plan also gives you a great opportunity to be involved in the decisions around what treatments, providers and goals you would like for your health.
To qualify for a Care Plan you must have a chronic condition such as asthma or diabetes for example and you must book an appointment with our nurse who will guide you through the plan. You can then discuss what services and providers may be most suitable to meet your needs.
